Consent Forms We must have a completed consent form for all participants on our activities. This folder contains both informed consent forms for all participants and parental consent forms for under 18's.

We must have a copy of this form for every participant under the age of 18. These forms must be completed and received by us at least one week before the start of the event.
This form must be filled in by all participants over 18 years of age (though we encourage under 18's to complete them too) on all activities run by lupine Adventure Co-operative.
